BLNRC is seeking a Full-Time Social Service Designee and Admissions Coordinator. Work schedule would be Mon-Friday, 8a-4:30p. This job role will be under the supervision of the Director of Social Worker.
This person is responsible for visiting with and assisting residents and their families who are interested in applying to live at Nelson Manor or BLNRC. They are also responsible for assisting in the identification of residents’ needs and problems, working with department supervisors and community resources to meet the needs of the residents at Bethel Lutheran Nursing & Rehabilitation Center. Requirements include:
- Two year of Social Service experience in a healthcare setting preferred but not required; LTC experience a plus.
- A general interest and ability to work with residents in a nursing home environment.
- Preferred education background in Social Work or related field.
- Bachelor’s or Master’s degree in Social Work not required, but must be licensed by the state of North Dakota if that degree is held.
Duties of Admissions Coordinator include, but are not limited to:
- Visiting with residents and families about interest in application for admission.
- Assist with completion of forms as needed and directed by supervisor.
- Home/hospital visits with potential residents if needed.
- Work with Direct of Nursing to determine appropriate level of care.
- Ensure all paperwork is received and distributed to appropriate departments as needed.
- Ensure PASRR screenings are completed and approved.
- Communicate information about new residents to appropriate departments.
Duties of Social Service Designee include, but are not limited to:
- Advocating for residents and their needs
- Completing social assessments
- Reviewing advance directive information, resident rights and ombudsman program with residents and their families
- Participate in MDS and other required assessments
- Updating medical records, progress notes and care plans as needed/required
- Working with residents, families and community resources to assist in referral service solutions inside or outside of the facility to meet resident needs
- Candidate will cross train for admissions process with current admission lead
Ideal candidate will have a love for the elderly, be a team player and willing to take direction and learn on the job.
