Social Service Designee - FT

BLNRC is seeking a Full-Time Social Service Designee. Work schedule would be Monday-Friday, 8:00am-4:30pm. All employees are required to complete orientation program upon employment.

Duties include but are not limited to, advocating for residents and their needs, completing social assessments, reviewing advance directive, resident rights, and ombudsman program with residents and their families, participate in MDS and other required assessments, updating medical records, progress notes and care plans as needed/required, and working with residents, families and community resources to assist in referral service solutions inside or outside of the facility to meet resident needs.

Candidate will cross train for admissions process with current admission lead.

Background check & drug screen required.
