Social Worker / Director of Social Services - FT

BLNRC is seeking a Full-Time Social Worker/Director of Social Services. Work schedule would be Monday-Friday, 8:00am-4:30pm.

This person is responsible for assisting in the identification of residents’ needs and problems, while working with department supervisors and community resources to meet the needs of the residents. Requirements include:

• Social Service experience in a healthcare setting preferred but not required; LTC experience a plus.

• Ideal candidate will have a love for the elderly, be a team player and be willing to take direction and learn on the job.

• An interest and ability to work with residents in a nursing home environment.

• Basic computer skills; specific software (i.e. record keeping) and facility process training will be provided.

• Strong customer service and communication skills a must.

• Willing to train the right candidate!

Duties include, but are not limited to:

• Complete social assessment on residents within 7 days of admission and yearly thereafter.

• Review advance directive information with resident/family/legal representative within 7 days of admission.

• Orally review Resident Rights and Ombudsman program within 30 days of admission and yearly thereafter and provide written copies.

• Participate in MDS, PASRR Screenings, and other assessments as needed.

• Provide supportive services to residents and their families.

• Provide/recommend referral services and counseling as needed.

• Coordinate and communicate room transfers, bed holds and discharge information.

• Coordinate family meetings, conferences and/or concerns.
